If you live in our catchment area then we are able to register you as a patient here. We accept new patient registrations on Mondays, Tuesdays (9 am to 5pm) and Wednesdays (9am to 6pm).

Please ensure documents are dated within the last 3 months. We accept the following forms of documentation:
council tax bill
fixed utility bill (not mobile phone)
tenancy agreement (signed and dated)
This is to protect local residents due to the demand for our services from people who do not live in the city centre.
Registration Forms:
We will need you to fill in two forms: a NHS registration form (GMS1) and our admin questionnaire. The forms can be downloaded below and brought in with you when you come to register.
Alternatively you can come into the practice and ask at our reception for these forms.

When you come to register:
We will book a Health Check appointment for you with one of our health care workers. This is an opportunity for us to understand your clinical needs and is an essential requirement to complete your registration. This appointment will either be on a Tuesday (afternoon) or a Wednesday (late afternoon or evening).
If you are taking medication at the moment, please bring evidence of that medication with you to your health check appointment.
For example,
your prescription repeat slip
the packing your medication came in.
If you have any other evidence of your medical history, such as a record of vaccinations, this would also be helpful as we will not receive your previous notes for some weeks. We will also provide you with a pot for a urine sample. This will then complete your registration.
